Tuberculosis, or TB, is an infectious bacterial disease caused by Mycobacterium tuberculosis, which most commonly affects the lungs. It is transmitted from person to person via droplets from the throat and lungs of people with the active respiratory disease. Approximately 90 percent of people infected with TB are not sick and cannot spread the infection to others, only 10 percent of those infected will become sick with tuberculosis (TB disease). 5% of these people are sick with TB within two years. Most of them are children and people with low immunity. While the remaining 5% will become sick with TB after many years of first being infected, such as elderly patients with a history of contact with tuberculosis as a child. By the nature of tuberculosis patients who are not treated, 30-40 percent will die within one year and 50-70 percent will die within 5-7 years. The symptoms of active TB of the lung are coughing, sometimes with sputum or blood, chest pains, weakness, weight loss, fever and night sweats. Risk factors for TB are reduced immunity in body, such as HIV infection, diabetes, malnutrition or receiving immune-suppressants. Today, HIV infection is the most important risk factor for TB infection. Thailand has implemented TB control guidelines of the World Health Organization (The internationally agreed strategy for TB control: DOTS strategy) since 1996 and updated the Stop TB Strategy in 2006, by further integrating TB and HIV issues, creating the care system of Multidrug- resistant TB (MDR-TB), strengthening health systems and promoting the participation of citizen education and research. Principles for the treatment of tuberculosis Short-term treatment with standard regimens that are currently being used have been internationally recognized as effective and efficient in the treatment of TB patients, with an almost 100 percent cure rate if the following precautions are taken: • Correct type and number of medicines. • Correct dosage of medicine • Duration of treatment is sufficient. Short term treatment has 3 courses of treatment 6, 8 and 9 months. Patients must fully complete the course of treatment or they may be a chance of recurrence, or may develop drug resistant TB. • Continuation of treatment.
